All Initial Consultations
Regardless of which practitioner you are attending, you will be asked to complete an Entry Form prior to your treatment. This will be provided to you on your arrival at the clinic.
For Naturopathy Consultations
Please complete a Health Appraisal prior to your appointment. This can be done in one of 2 ways.
- Shorter version. This is for people with a few health problems and symptoms only. Please download your Health Appraisal. Please complete and bring this with you to your appointment.
Or
- In depth Health Appraisal. Please complete this form if you have several health concerns or symptoms. This on-line format will take approximately ½ hour to complete. The form will be retrieved off of the internet by us, prior to your visit (no need to try and fax or email).

Preparing for Tests
Several tests are performed with your standard consultation. In most Initial Consultations we will complete
1) Urinalysis – no preparation is required
2) Urinary Indicans test – normally recommended if there are any gut, immune or skin
problems.
Preparation (for more accurate result) – no alcohol for 24 hours
- some protein in the diet the night before your consultation
3) Cholesterol and Fasting Blood Sugar levels – nil by mouth, except for water, for 12 hours
prior to testing
Cancellation Policy
Please ensure that you advise the clinic when you are unable to attend your appointment. We understand that occasionally this is not possible and that everyone forgets now and again. To ensure we are able to book as many people into their desired appointment times however, notification is appreciated.
Remember that failure to attend your follow up appointments is detrimental to your ongoing care. Initial Consultation generally aims to provide symptomatic relief, whilst follow up appointments address the underlying causative health triggers to ensure your health problem is resolved properly.
A series of “no shows” (when a person does not cancel and fails to attend an appointment) may result in our refusal to continue with your care.

HICAPS
A HICAPS facility is available at our clinic. HICAPS allows you to receive an automatic refund through your Private Health Insurer (as long as your Private Health Insurer is connected to the HICAPS scheme). If your provider is not connected to HICAPS, you will be given a printed receipt which you must submit to your Private Health Insurer for your refund.
